Five Available Tabs

The following tabs act as steps when creating a survey.

  1. Survey Designer Tab
  2. Survey Settings Tab
  3. Survey Default Tab
  4. Survey Layout Tab
  5. Negative Feedback Notification Tab


Some of these tabs may not display if certain settings are chosen.


1) Survey Designer

The Survey Designer tab many different options for creating the survey. The first step entails entering the Survey Title and Survey Introduction text i.e. Please take our survey to earn 25 Bonus Points!


If using the default survey, which includes category questions that are grouped together on the Survey Results page, there is no need to do anything on this page outside of entering the Survey Title and Introduction Text, and selecting 5-Star Scoring or A-F Scoring. You can, however, ask additional questions in addition to the category questions or remove the category questions altogether. The survey category questions are found on the Survey Default tab.


If using additional questions outside of the category questions, when viewing survey results, they must be reviewed one at a time. You can learn more about reviewing surveys in the Survey Results Article.


Drag & Drop Questions

The widget column on the left contains setting features and drag and drop question functionalities.

  • 5 Star Scoring: Select if you would like to use 5-star scoring.
  • A-F Scoring: Select if you would like to use A-F scoring.
  • Do Not Ask: Select if you want to create a custom survey with no categories.
  • Single Line of Text: Drag/Drop for a single line of text.
  • Paragraph of Text: Drag/Drop for a paragraph of text.
  • List of Radio Buttons: Drag/Drop for a question with radio buttons.
  • List of Checkboxes: Drag/Drop for a question with checkboxes
  • Drop Down List: Drag/Drop for a question with a drop down list.
  • Drop Down List of Countries: Drag/Drop for a question with drop down country selection.
  • File Upload Box: Drag/Drop to add a file upload box.
  • Receipt Upload: Drag/Drop to add a receipt upload box. Note, this feature is not available for all programs. If adding a receipt upload box, transactions will be posted by the members mobile phone or web browser via uploading receipts found inside the members profile. The transactions display as "Pending" in on the View Transactions page. To enable this feature, create a support ticket or contact your account representative.
  • Section Break: Drag/Drop to add a section break.


When finished adding questions, press the Survey Settings tab.


2) Survey Settings

The Survey Settings tab has two separate sections.


Display & Feedback Options

  • Change Default Logo: If you would like to change the default banner do so here.
  • When the Survey is Completed: There are four options.
    1. Display the text in field box on a thank you page.
    2. Take the member to a particular web page by entering a web URL.
    3. Take them to a Refer a Friend page for viral growth
    4. If positive rules score then, you may display a popup with social citation links for additional reviews. This is the most common setting used. When clicked, select the score required to display the citation link. Enter the citation links. To add more links press the “Add New” button and to apply the changes in the HTML editor press the “Apply Changes” button. These citation links will display in the editor, and the text can be manually changed. If the member does not meet the positive rules, create a default thank you message for taking the survey.
  • Email Survey Responses: If you would like the survey responses emailed, check this box and enter your email.


Advanced Options

  • Error Message: If there’s an error message when the surveys taken, enter the message here.
  • Survey Link Clicked After Completion: If a survey has been taken and the Submit Button is pressed a second time, this message will display. Feel free to customize the message to your needs.
  • Submit Form: If you would like to change the default Submit Form button text, do so here.


When finished with this tab, go to the Survey Default tab.


3) Survey Default

The first five questions on this page are ranked using a Five-Star or A-F scoring system depending on the option chosen in step 1. Category questions display in an organized fashion on the survey results page allowing the business owner to get an overall snapshot of the business’s performance. For each question you have the following options:

  • Ask this Question? Choose whether to ask the category question.
  • Category: Choose appropriate category names.
  • Question: Enter the question you would like to ask.
  • Description: Use the default description or create your own.


The 5th question titled “Server Question” has two additional options:

  • Question: Choose to ask something about the server.
  • Description: Choose to use the default description or create your own.


Question #6-8 are additional questions that can be asked to get members written feedback. Choose to ask a positive question, a negative question, and a suggestion question. If asking the negative feedback question and the member completes the field, a notification will be sent to the business owner. Typically, this question is turned "Off" now that we’ve added the Negative Feedback Notification Tab. However, there may be certain instances where this may be favorable.


When finished with this tab, go to the Survey Layout tab.


4) Survey Layout

The survey layout tab allows you to customize the look and feel of the survey. Without going through each and every customization field, you may change text font sizes and colors, paragraph widths and heights, border colors, button colors, drop shadow options and we have an entire page background library of images including pattern overlay images.


We recommend saving the survey and keeping this page and the view survey page open simultaneously to view changes. When making changes in one of the fields on this page, refresh the view survey page and see the results.


When finished with this tab, go to the Negative Feedback Notification tab.


5) Negative Feedback Notification

The negative feedback notifications settings allow you to receive an email or text message if certain survey scores rules are reached. By default, 3.0 is in place for the five-star scoring and "C" is in place for the A-F scoring. Therefore, if 3.0 stars or "C" scores or less are received in any one category, an email or SMS will be sent if the checkbox, “Yes send me an email to these addresses” or “Yes, send me an SMS to this mobile phone number” is checked.


When finished with all the tabs settings, press the Save button.
